Care Plans
Care plans help outline what is needed to manage any chronic diseases that you may have over a 12 month period. This could be either a medical or mental health care plan.
A Chronic Disease Care Plan helps your doctor and you set goals to manage your condition/s by utilising a team care approach. You will be entitled to 5 visits to different allied health services each year.
A Diabetes Cycle of Care is done for diabetic patients to make sure their condition is well managed. This is done throughout the year.
A Mental Health Care Plan enables you to access mental health services to help manage your mental disorder. It entitles you to 10 individual and 10 group appointment to certain psychologists, occupational therapists and social workers each year.
We also do Advanced Care Planning for our elderly patients to prepare them and their family for their future health care. It is a good way for them to let their loved ones know how they will be treated if they are not able to communicate or participate in making that decision.
Ask your doctor is you are eligible for any of the above care plans. You will then be booked in for a long appointment to get this organised and done.
